    Update! APD Needs Assessments

    The Agency For Persons With Disabilities (A.P.D.) plans to complete a QSI (Questionnaire for Situational Information) for all individuals on waiver programs over the next two years. The QSI Assessments were mandated by the Florida Legislature and is one of the responsibilities of receiving waiver funded services. The QSI is the new needs assessment tool being used by A.P.D. Prior to the QSI, Similar needs assessments have been done by A.P.D. The FSTS and ICG were used in the past. The new QSI assessments will be done by A.P.D. employees, rather than support coordinators.

    It's easy to search for medicaid waiver providers. To make it easier to find providers in your area, there are symbols next to each provider's name, showing the districts they serve. You will learn a lot about providers by looking at their descriptions and visiting their websites.

    Question: How Do I Get On The Medicaid Waiver?

    Answer: You need to fill out the application for services. Florida's HCBS and FSL waiver programs are for people who have certain disabilities. These disabilities include: Mental Retardation, Spina Bifida, Cerebral Palsy, Prader-Willi, and Autism. For More Information, you can call A.P.D.'s Toll Free Number 1-866-APD-CARES or

    Question: Can I choose anyone I want to be my provider?

    Answer: To a large degree, you have choice of providers. However, the provider must serve the waiver you are on. The provider must also be certified to work in your area and for the services you choose. You can not choose a provider that is your legal guardian or parent.

    Question: Can I get everything the waiver offers?

    Answer: No, you need to work with you support coordinator and determine what services and products are "medically necessary".

    Question: I have been on the wait list for several years. How can I get on the waiver faster.

    Answer: There are currently over 15,000 people waiting for waiver services in Florida. The only way to get on the waiver faster is by being in crisis. Examples of crisis include: homeless, no suitable caregiver, and extreme behaviors. Being in crisis does not guarantee waiver placement.

    What Are The FSL And HCBS Waivers?

    The waivers cover things that medicaid typically does not cover.

    The HCBS/DD Waiver (Now Tiers 1, 2, and 3)
    • Also know as the "Big Waiver"
    • Also know as the Home and Community Based, Developmental Disabilities Waiver
    • This Waiver currently does not have a spending cap. There are 28 services, learn about them all.
    The FSL Waiver (Now Tier 4)
    • Also know as the "Little Waiver"
    • Also know as the Florida Supported Living Waiver
    • Also know as Tier 4
    • This Waiver currently has a spending cap of $14,792.00.
    • There are 11 services, learn about them all.

Come back often, we update our site every week. We are always adding new resources and current information. Remember that while we are working to get providers listed- letting them know about our services and building them websites- we do offer a free service to support coordinators. We will call providers for you. However, due to HIPPA rules, we can only call for support coordinators.

    Waiver Providers, you can add your listing even if you don't have a website. We provide a free one page "web flyer" and free hosting, with your membership.

    Question: How Do I Become A Provider?

    Answer: In most cases, we would recommend that you start by working as a provider for another company for at least 6 months. This is because the paperwork requirements are vast, confusing, and necessary to get paid.

    If an individual would like to become a Developmental Disabilities Waiver provider, they may contact one of the Provider Enrollment Specialists located at the Area APD offices. The contact information can be found at http://www.apd.myflorida.com/area. The provider will need to complete a Florida Medicaid Provider Enrollment Application and a Provider Supplement. These forms can be located on the APD website at http://apd.myflorida.com/waiver under the heading "Home and Community Based Waiver (HCBS) - Provider Application." The Area APD Provider Enrollment Specialist will work with the provider to identify other information that may be needed, depending on the service. Each provider must meet background screening requirements. This will include a check by local law enforcement, FBI, FDLE, employment and personal reference checks. The provider must submit a notarized Affidavit of Good Moral Character Form, which they may obtain from the APD Enrollment Specialist or the Department of Children and Families while undergoing background screening. Providers must demonstrate that they meet the minimum qualifications and training requirements for the services that they provide as identified in the Developmental Disabilities Waiver Handbook. This handbook can be found HERE . These requirements vary depending on the service for which the applicant is seeking enrollment. I encourage any person who is interested in providing services to contact the Area Provider Enrollment Specialist for assistance.
